The Toyota, driven by Kody McIntire, 20, of Alton, was northbound on Highway 63. According to the investigating trooper with the Arkansas State Police, Cpl. Ronnie D. Stewart, McIntire crossed the center line into the southbound lane.
Stewart's report said McIntire's vehicle crashed head-on with Daniel Lash, 52, of Salem, driving the tractor trailer, which was heading south on the highway.

Lash was transported by private vehicle to Fulton County Hospital for treatment of minor injuries and was released.
Stewart reported that the Mammoth Spring First Responders, Mammoth Spring Volunteer Fire Department, Thayer Fire and First Responders, Arkansas Highway Police, Mammoth Spring Police Department, Oregon County Ambulance and Fulton County Ambulance all responded to the accident.
